Background:

  • Primary hypophysitis (PH) is a rare inflammatory condition affecting the pituitary gland.
  • Understanding its clinical presentation, diagnostic challenges, and treatment outcomes is crucial for patient management.

Purpose of the Study:

  • To review the clinical outcomes of patients diagnosed with primary hypophysitis (PH).
  • To assess the effectiveness and safety of various treatment modalities for PH.

Main Methods:

  • Retrospective evaluation of 17 patients with PH followed between 2007 and 2018.
  • Assessment of clinical, endocrinologic, pathologic, and radiologic findings.
  • Analysis of treatment modalities including observation, surgery, and steroid therapy.

Main Results:

  • Headache and anterior pituitary deficiencies (hypocortisolemia, hypothyroidism) were common symptoms.
  • Radiologic findings varied, including space-occupying lesions and loss of the posterior pituitary bright spot.
  • Observation was the most frequent initial treatment, with 80% of patients showing no endocrinologic deterioration. Steroid treatment in 5 patients led to serious adverse effects in 60%.

Conclusions:

  • Accurate diagnosis and tailored treatment for PH remain challenging.
  • The efficacy of active steroid treatment for PH is uncertain, and it is associated with significant adverse events.
  • Observation, surgery, and radiotherapy are viable treatment options for selected PH patients.
