Purpose of the Study:

  • To investigate in vivo the direct communication between cerebrospinal fluid (CSF) and the extravascular compartment of human visual pathway structures.
  • To determine if CSF can access the optic nerve, optic chiasm, optic tract, and visual cortex.

Main Methods:

  • A prospective, observational study involving 10 subjects undergoing intrathecal gadolinium-enhanced MRI.
  • Gadobutrol was administered intrathecally as a CSF tracer, with MRI scans acquired over 24-48 hours.
  • Regions of interest were analyzed for contrast enrichment along the visual pathway, from the visual cortex to the vitreous body.

Main Results:

  • Significant CSF tracer enrichment was observed in the optic nerve, optic chiasm, optic tract, and primary visual cortex (P < 0.001).
  • Peak tracer enhancement in the visual pathway occurred after 24 hours, following enhancement in the prechiasmatic cistern (4-6 hours).

  • Direct communication exists between the CSF and the extravascular space of the human visual pathway.
  • These findings suggest the presence of a glymphatic system within the human visual pathway.
  • The identified glymphatic system may offer novel perspectives on the pathophysiology and treatment of ophthalmic diseases.
