Relationship Between Exercise Oscillatory Ventilation Loop and Prognosis of Heart Failure

Exercise oscillatory ventilation (EOV) loop size predicts cardiovascular death in heart failure patients. Larger EOV loop size and lower hemoglobin levels are associated with increased mortality risk.

- Exercise oscillatory ventilation (EOV) is a phenomenon observed during CPX, but its prognostic value in heart failure (HF) is understudied.
- EOV severity can be quantified using end-tidal CO2 pressure and ventilation patterns.
Purpose of the Study:
- To evaluate the association between EOV severity and prognosis in patients with heart failure with reduced ejection fraction (HFrEF).
- To determine if EOV severity predicts cardiovascular death in HFrEF patients.
Main Methods:
- Retrospective analysis of 2,043 patients undergoing CPX.
- Inclusion of 70 HFrEF patients with EOV.
- Median follow-up of 4.3 years with cardiovascular death as the endpoint.
Main Results:
- 48% of participants (34/70) died during follow-up.
- Deceased patients had significantly larger EOV loop size (17.3±7.0 cm² vs. 12.8±6.1 cm², P<0.001).
- Lower hemoglobin (Hb) levels were also observed in patients who died (12.2±1.2 g/dL vs. 13.2±2.9 g/dL, P=0.004).
Conclusions:
- EOV loop size is a significant predictor of cardiovascular death in HFrEF patients.
- Both EOV loop size and Hb levels are independent predictors of mortality in this population.
