Background:

  • Common variable immunodeficiency (CVID) is a primary immunodeficiency marked by poor antibody response and recurrent infections.
  • CVID patients often exhibit non-infectious inflammatory and autoimmune issues linked to immune system overactivation.

Purpose of the Study:

  • To investigate if High-Density Lipoprotein (HDL) function is impaired in CVID patients.
  • To determine if impaired HDL contributes to the inflammatory phenotype observed in CVID.

Main Methods:

  • Compared HDL cholesterol levels in CVID patients and healthy controls.
  • Assessed reverse cholesterol transport and cholesterol efflux capacity in CVID patients.
  • Evaluated HDL's ability to suppress TNF release in response to TLR2 stimulation in CVID patient cells.

Main Results:

  • CVID patients, especially those with inflammatory/autoimmune complications, had lower HDL cholesterol levels.
  • Reduced HDL cholesterol correlated negatively with inflammatory markers CRP and sCD25.
  • Impaired cholesterol efflux capacity and reduced ABCA1 expression were observed in CVID patients.
  • HDL from CVID patients showed diminished suppression of TNF release and decreased ATF3 expression.

Conclusions:

  • Impaired HDL function is linked to systemic inflammation in Common Variable Immunodeficiency (CVID).
  • These findings highlight HDL as a potential therapeutic target for CVID and other inflammatory or autoimmune diseases.
