Summary

This study shows that a comprehensive rehabilitation program can improve serum C-terminal telopeptide of type II collagen (CTX-II) levels and enhance the quality of life for patients suffering from knee osteoarthritis (KOA).

Background:

  • Knee osteoarthritis (KOA) is a prevalent, progressive, and incurable joint disease causing pain, disability, and significant socioeconomic burden.
  • Cartilage and bone degradation are hallmarks of KOA, impacting patient quality of life.

Purpose of the Study:

  • To investigate changes in serum C-terminal telopeptide of type II collagen (sCTX-II) before and after a complex rehabilitation program in symptomatic KOA patients.
  • To correlate clinical, functional, sCTX-II, and histological parameters in KOA patients undergoing rehabilitation.

Main Methods:

  • An observational study involving 24 KOA patients (aged 59-76) assessed at baseline (T1) and four months post-rehabilitation (T2).
  • Evaluated parameters included pain (Visual Analogue Scale), stiffness and physical function (WOMAC), sCTX-II (ELISA), and arthroscopic cartilage histology.
  • Histological analysis revealed superficial cell changes, reduced proteoglycans/glycosaminoglycans, and matrix degeneration.

Main Results:

  • A complex rehabilitation program led to improvements in sCTX-II values.
  • Patients experienced enhanced quality of life following the rehabilitation intervention.
  • Positive correlations were observed between clinical improvements and sCTX-II changes.

Conclusions:

  • Complete treatment, including rehabilitation, is beneficial for improving sCTX-II levels in KOA patients.
  • Rehabilitation programs positively impact the quality of life for individuals with knee osteoarthritis.
  • sCTX-II may serve as a useful biomarker for monitoring treatment response in KOA.
