  • A 62-year-old woman presented with an enlarged tongue, incomprehensible speech, sialorrhea, and chewing difficulties.
  • No other causes for macroglossia were identified.
  • Nocturnal hypercapnia and desaturation were observed, attributed to macroglossia.

Findings:

  • The patient was diagnosed with early-stage motor neuron disease, likely PLS.
  • Non-invasive ventilation (NIV) with pressure control mode was initiated.
  • Post-NIV capnography showed normalization of end-tidal carbon dioxide (EtCO2) levels.

Implications:

  • This is the first reported case of macroglossia in primary lateral sclerosis.
  • Further research is needed to understand the pathogenesis of macroglossia in PLS.
  • NIV may be a beneficial intervention for respiratory complications associated with macroglossia in PLS.
