Summary
Salvage lymph node dissection (SLND) offers limited oncological benefit for hormone-naïve prostate cancer patients with nodal recurrence. Prostate-specific membrane antigen (PSMA) PET/CT scans are currently too imprecise for diagnosing metastatic sites in these patients.
Area of Science:
- Urology
- Oncology
- Nuclear Medicine
Background:
- Nodal recurrence of prostate cancer after radical prostatectomy presents a challenge.
- Salvage lymph node dissection (SLND) is a debated treatment option for these patients.
- Accurate staging is crucial for treatment planning.
Purpose of the Study:
- To assess the oncological benefit of SLND in hormone-naïve patients with recurrent prostate cancer.
- To evaluate the diagnostic accuracy of prostate-specific membrane antigen (PSMA) positron emission tomography-computed tomography (PET/CT) for detecting nodal recurrence.
- To determine if SLND can delay the need for further hormonal therapy.
Main Methods:
- Retrospective analysis of 43 hormone-naïve men who underwent transperitoneal SLND.
- Oncological outcomes assessed via serum prostate-specific antigen (PSA) testing.
- Diagnostic accuracy of PSMA PET/CT evaluated using sensitivity, specificity, PPV, and NPV.
Main Results:
- Only 18.6% of patients achieved complete biochemical response post-SLND.
- Median time to biochemical recurrence was 2 months.
- PSMA PET/CT showed low sensitivity (32%) and moderate specificity (91.74%) for detecting metastatic sites.
Conclusions:
- Transperitoneal SLND provides limited oncological benefit for hormone-naïve prostate cancer patients with nodal recurrence.
- SLND does not appear to be an effective treatment for cure or to delay salvage hormone manipulation.
- Current PSMA PET/CT imaging is too imprecise for diagnosing metastatic disease in this patient cohort.

Detailed Structure and Function of Lymph Nodes
4.5K
Lymph nodes are bean-shaped structures that cluster along the lymphatic vessels in the inguinal, axillary, and cervical regions. Each node is divided into compartments by a capsule that extends trabeculae inward.
From a histological perspective, lymph nodes can be split into two main areas: the superficial cortex and the deep medulla. The outer cortex is populated by dendritic cells, macrophages, and B lymphocytes, which are densely packed into follicles. When these B-lymphocytes are presented...

Plant Hormones
27.4K
Plant hormones—or phytohormones—are chemical molecules that modulate one or more physiological processes of a plant. In animals, hormones are often produced in specific glands and circulated via the circulatory system. However, plants lack hormone-producing glands.

Types of Hormones
83.1K
Hormones can be classified into three main types based on their chemical structures: steroids, peptides, and amines. Their actions are mediated by the specific receptors they bind to on target cells.

Types of Hormones

Hormones are classified into four main groups: steroids, eicosanoids, amino acid-based derivatives, and peptide hormones.
Steroids and eicosanoids fall under the category of lipid-soluble hormones. Steroids are derived from cholesterol and feature four interconnected carbon rings with variable side chains. Notable examples include estradiol from ovaries and testosterone from testes, exemplifying the critical roles of these lipid-soluble hormones in reproductive physiology. Eicosanoids, derived...

Hormonal Regulation
35.8K
The renin-aldosterone system is an endocrine system which guides the renal absorption of water and electrolytes, thus managing blood pressure and osmoregulation. Activation of the system begins in the kidneys with a small cluster of cells adjacent to the afferent and efferent blood vessels of the renal corpuscle. As the nephrons are filtering blood, juxtaglomerular cells monitor blood pressure. If they detect a decrease in pressure, they release the hormone renin into the bloodstream.

Hormonal Regulation
47.9K
Hormones regulate a significant portion of digestion through activation of the neuroendocrine system. The neuroendocrine system of digestion contains many different hormones all with multiple functions that are both, directly and indirectly, involved in digestion.
