Summary

Magnetic Resonance Elastography (MRE) shows higher accuracy than 2D-Shear Wave Elastography (2D-SWE) for detecting significant fibrosis in chronic hepatitis B (CHB) patients. MRE aids in deciding on antiviral therapy for CHB patients with normal or mildly elevated ALT levels.

Background:

  • Accurate detection of significant fibrosis (≥ F2) is crucial for initiating antiviral therapy in chronic hepatitis B (CHB) patients, particularly those with high viral loads and normal or mildly elevated ALT.
  • Liver biopsy, the gold standard, is invasive; thus, noninvasive imaging techniques like MRE and 2D-SWE are explored for fibrosis assessment.

Purpose of the Study:

  • To evaluate the efficacy of MRE and 2D-SWE in identifying liver fibrosis stages in CHB patients.
  • To compare the diagnostic performance of MRE and 2D-SWE against serum-based indices (FIB-4, APRI).

Main Methods:

  • Sixty-three treatment-naïve CHB patients with high viral loads and normal/mildly elevated ALT underwent liver biopsy, MRE, and 2D-SWE.
  • Serum-based indices FIB-4 and APRI were calculated.
  • Diagnostic performance was assessed using receiver operating characteristic (ROC) analysis with liver histology as the reference.

Main Results:

  • MRE significantly differentiated fibrosis stages (F2 vs. F0/1; P=0.022) with better correlation to fibrosis stage (r=0.869) than 2D-SWE (r=0.649).
  • MRE demonstrated superior diagnostic performance for significant fibrosis (AUC=0.906) compared to FIB-4 (AUC=0.697) and APRI (AUC=0.717).
  • 2D-SWE (AUC=0.843) showed no significant performance difference from FIB-4 or APRI.

Conclusions:

  • MRE offers a more precise noninvasive assessment of significant fibrosis in CHB patients compared to 2D-SWE.
  • MRE can aid in treatment decisions regarding antiviral therapy initiation for CHB patients with normal or mildly elevated ALT levels.
