Summary

Imported cutaneous diphtheria (a skin infection caused by Corynebacterium diphtheriae) was diagnosed in two travelers returning from tropical regions. Prompt antibiotic treatment led to complete healing of the skin ulcers in both cases.

Area of Science:

  • Infectious Diseases
  • Dermatology
  • Tropical Medicine

Background:

  • Cutaneous diphtheria is a bacterial skin infection caused by toxin-producing Corynebacterium diphtheriae.
  • While typically associated with endemic regions, imported cases can occur in non-endemic areas.

Observation:

  • Two cases of imported cutaneous diphtheria are presented in travelers returning from tropical locations (Seychelles and Southeast Asia/Nepal).
  • Both patients presented with non-healing skin ulcers, one on the malleolus and the other on the foot, with no signs of systemic illness.
  • Microbiological investigation confirmed the presence of toxinogenic Corynebacterium diphtheriae in both cases.

Findings:

  • The causative agent was identified as toxinogenic Corynebacterium diphtheriae in both imported cutaneous diphtheria cases.

Related Experiment Videos

  • Neither patient exhibited signs of systemic diphtheria intoxication, indicating localized infection.
  • Treatment with erythromycin resulted in complete resolution of the cutaneous lesions in both individuals.
  • Implications:

    • These cases highlight the potential for imported cutaneous diphtheria in travelers returning from endemic or tropical areas.
    • Early diagnosis and appropriate antibiotic treatment (erythromycin) are crucial for successful management and preventing complications.
    • Public health awareness regarding diphtheria in travelers may be warranted, especially for those with incomplete vaccination histories.
