Background:

Knee joint pain can present diagnostic challenges.
Magnetic resonance imaging (MRI) and clinical examination may not always identify the source of pain.
Medial meniscus hypermobility is a potential, though less common, cause of knee pain.

Observation:

A young college soccer player experienced persistent knee joint pain.
Preoperative assessments, including clinical examination and MRI, failed to yield a diagnosis.
Arthroscopy revealed medial meniscus hypermobility causing femorotibial joint impingement.

Findings:

The medial meniscus was hypermobile but did not have a substantial tear.
An inside-to-outside meniscus suture technique was employed.
Post-suture, the medial meniscus hypermobility was resolved.

Implications:

Arthroscopic inspection is crucial for diagnosing subtle meniscal abnormalities like hypermobility.
Meniscal suture can effectively treat symptomatic medial meniscus hypermobility.
Successful surgical intervention enables athletes to return to sport without pain.
