Summary

A retained peripheral nerve block catheter (PNBC) caused a rare complication in a patient treated for phantom limb pain. A 15cm segment of the sciatic nerve catheter was discovered and removed months after initial placement.

Background:

Peripheral nerve block catheters (PNBCs) are used for continuous pain management.
Postamputation phantom limb pain can be intractable and challenging to treat.
Continuous infusions via femoral and sciatic PNBCs were administered to a patient with severe pain.

Observation:

The patient presented with a discharging sinus from the sciatic nerve catheter site five months post-procedure.
Initial magnetic resonance imaging (MRI) was inconclusive in identifying the retained catheter.
Surgical exploration was required to diagnose and address the complication.

Findings:

A significant 15cm segment of the peripheral nerve block catheter was found to be retained within the sciatic nerve.
The retained catheter segment was successfully removed during surgical exploration.
This case highlights a rare but serious complication associated with PNBCs.

Implications:

Delayed diagnosis and management of retained PNBCs can lead to infection and further complications.
This case underscores the importance of vigilance and thorough examination during catheter removal.
Improved imaging or diagnostic techniques may be beneficial in identifying retained foreign bodies in nerve blocks.
Consideration of PNBC material and securement methods could potentially reduce the risk of fragment retention.
