    Summary

    A novel technique using internal limiting membrane (ILM) scaffolding effectively closes large macular holes (MHs) through photoreceptor migration. This method offers a promising approach for challenging MH cases, leading to improved visual acuity.

    • Existing techniques for MH closure have limitations, particularly for large or complex cases.

    • A new surgical technique is described for closing challenging macular holes.
    • The procedure involves vitrectomy, internal limiting membrane (ILM) peeling, and macular isolation under perfluorocarbon liquid.
    • Fluid aspiration from the MH and sealing with an inverted ILM flap or autologous ILM free flap are key steps.

    Findings:

    • ILM scaffolding over the surgically reduced MH promotes closure via photoreceptor migration, not gliosis.
    • The technique achieved intraoperative closure of MHs.
