Antimicrobial computerized decision support systems (CDSS) aim to enhance antibiotic prescribing.
However, the adoption and proven effectiveness of these systems remain limited.

Purpose of the Study:

To review existing evidence on the impact of CDSS on antimicrobial prescribing.
To discuss challenges in assessing CDSS effectiveness in hospital and primary care settings.

Main Methods:

Literature search to identify studies on CDSS and antimicrobial prescribing.
Analysis of evidence regarding CDSS use in hospital and primary care settings.

Main Results:

Evidence for clear benefits of CDSS on antimicrobial outcomes is scarce.
CDSS shows a modest impact on antibiotic prescribing in primary care, potentially due to underutilization.

Conclusions:

Low uptake and limited evidence highlight challenges in implementing antimicrobial CDSS.
Further investigation into outcome measures and utilization is crucial for optimizing CDSS in antimicrobial stewardship.
