Chronic Bowel Disorders: Introduction01:17

Chronic Bowel Disorders: Introduction

Chronic bowel diseases are a group of long-term conditions affecting the digestive tract, characterized by inflammation and damage to the gut lining. These conditions primarily include irritable bowel syndrome and inflammatory bowel disease.
Irritable Bowel Syndrome (IBS) is a common disorder affecting the gastrointestinal tract. The distinctive feature is recurrent abdominal pain associated with altered bowel movements, manifesting as constipation, diarrhea, or fluctuating between both. The...

Endoscopic Procedures V: ERCP

Endoscopic Retrograde Cholangiopancreatography (ERCP) is a diagnostic procedure that combines endoscopy and fluoroscopy to diagnose and treat conditions related to the bile ducts, pancreatic ducts, and gallbladder. This procedure is beneficial for identifying and addressing blockages, gallstones, strictures, and tumors within the biliary or pancreatic systems. ERCP is both diagnostic and therapeutic, offering the ability to visualize and treat identified problems in one session.

Peptic Ulcer Disease III: Clinical Manifestations and Complications

Duodenal UlcersDuodenal ulcers are the most common form of peptic ulcer disease, presenting with chronic, intermittent epigastric pain. Pain typically appears 2–3 hours after meals, especially when the stomach is empty, often waking patients at night. It is characteristically relieved by food or antacids (“pain–food–relief”). Some patients remain asymptomatic until complications like bleeding or perforation emerge, particularly with NSAID or anticoagulant use.Gastric UlcersGastric ulcers share...
Diverticular Disease of the Colon01:27

Diverticular Disease of the Colon

Diverticular disease involves the formation of diverticula—small sac-like outpouchings of the colonic wall—and their complications. It most commonly affects the sigmoid colon due to higher intraluminal pressure and structural vulnerability. It results from structural weakness and increased pressure in the colon, producing pseudodiverticula that may remain silent or progress to inflammation and serious complications.Structure of DiverticulaIn diverticulosis, these outpouchings are...
Appendicitis01:19

Appendicitis

Appendicitis is an acute inflammatory condition of the vermiform appendix, most commonly caused by obstruction of its lumen. The appendix is a narrow, blind-ended pouch that extends from the cecum, making it particularly prone to obstruction. Causes include fecaliths, lymphoid hyperplasia (often after viral infections), parasites, tumors, or foreign bodies. This obstruction initiates a cascade of pathological changes.Luminal Obstruction and Early InflammationAfter obstruction, normal mucosal...
Cholecystitis01:20

Cholecystitis

Cholecystitis is inflammation of the gallbladder, most commonly caused by obstruction of the cystic duct. This blockage prevents bile from draining, leading to gallbladder distension, inflammation, and potentially serious complications. This condition may present acutely or chronically and can happen with or without gallstones.EtiologyAbout 95% of cholecystitis cases are calculous, caused by gallstones blocking the cystic duct, leading to bile accumulation and inflammation of the gallbladder...

Summary

Juxtapapillary duodenal diverticula are significantly associated with common bile duct stones. Patients with choledocholithiasis were 2.6 times more likely to have these diverticula, indicating a potential link.

Area of Science:

  • Gastroenterology
  • Hepatobiliary Medicine
  • Endoscopic Retrograde Cholangiopancreatography (ERCP)

Background:

  • Juxtapapillary duodenal diverticula are common findings during ERCP.
  • The relationship between these diverticula and choledocholithiasis requires further investigation.

Purpose of the Study:

  • To determine if juxtapapillary duodenal diverticula are associated with choledocholithiasis.

Main Methods:

  • Analysis of 250 consecutive ERCP examinations.
  • Assessment of cholangiography findings for common bile duct stones.
  • Statistical comparison of diverticula prevalence between patients with and without choledocholithiasis.

Main Results:

  • 35% of patients with common bile duct stones had juxtapapillary diverticula, compared to 12% of those with clear bile ducts (p<0.05).

  • Patients with choledocholithiasis were 2.6 times more likely to have periampullary diverticula.
  • A significant association was observed between periampullary duodenal diverticula and choledocholithiasis.
  • Conclusions:

    • Juxtapapillary duodenal diverticula are significantly associated with choledocholithiasis.
    • The presence of these diverticula may be a risk factor or indicator for common bile duct stones.
