Background:

The opioid epidemic has led to increased loperamide misuse.
Loperamide overdose presents significant cardiac risks, necessitating emergency department (ED) awareness.

Observation:

A 35-year-old male with long QT syndrome experienced severe arrhythmias and electrolyte disturbances after a loperamide overdose.
The patient required multiple defibrillator shocks due to the overdose complications.

Findings:

Loperamide overdose can precipitate life-threatening cardiac arrhythmias.
Underlying conditions like long QT syndrome exacerbate cardiac risks from loperamide.

Implications:

Emergency physicians need heightened awareness of loperamide overdose presentations.
Prompt recognition and management are crucial for mitigating severe cardiac events in loperamide toxicity.
