Background:

  • Wolfram syndrome is a rare genetic disorder causing diabetes, vision loss, and neurodegeneration.
  • Sleep disturbances are common in Wolfram syndrome but lack objective sleep study data.
  • Previous research has not objectively assessed sleep apnea in this population.

Purpose of the Study:

  • To evaluate the prevalence of sleep apnea in Wolfram syndrome patients.
  • To assess objective and self-reported sleep quality using validated tools.
  • To correlate sleep pathology with clinical and neuroimaging findings.

Main Methods:

  • Actigraphy and ambulatory polysomnography were used in genetically confirmed Wolfram syndrome patients.
  • Patients completed sleep quality questionnaires (ESS, PSQI, PSQ).
  • Clinical severity (WURS) and brain volumes (MRI) were assessed and compared to controls.

Main Results:

  • Obstructive sleep apnea (OSA) was highly prevalent: 29% in adults and 100% in children (≤12 years).
  • Higher apnea-hypopnea index (AHI) correlated with increased disease severity (WURS Physical scores).
  • Mixed apnea scores linked to reduced brainstem and cerebellar volumes; PSQ scores indicated worse childhood sleep-related breathing.

Conclusions:

  • Wolfram syndrome is associated with a high incidence of obstructive sleep apnea (OSA).
  • Further longitudinal studies are needed to understand the progression of sleep disorders.
  • Intervention for sleep disorders may significantly enhance the well-being of Wolfram syndrome patients.
