Background:

  • Intramedullary (IM) nailing is a standard orthopedic procedure for fractures and osteotomies.
  • Customization of IM nails is often required for optimal alignment, especially in complex deformity corrections.
  • A novel sterile bending device was developed for intraoperative IM nail customization.

Purpose of the Study:

  • To evaluate the technical aspects, indications, and limitations of intraoperative IM nail bending.
  • To assess the precision and safety of using a sterile bending device for IM nail customization in deformity correction.

Main Methods:

  • A retrospective clinical study involving 41 cases of deformity correction using intraoperatively bent IM nails.
  • Radiological outcomes of 31 completed cases were analyzed, comparing preoperative plans to postoperative results using long-standing radiographs.
  • Implant diameters ranged from 8.5mm to 13mm; cases involving fracture or non-union treatment were analyzed separately.

Main Results:

  • No signs of material fatigue were observed in removed implants after bending (1° to 12°).
  • High precision was achieved, with a median postoperative axis deviation of 3.5mm from the preoperative plan.
  • One case of an undisplaced cortical crack occurred during implant removal in a patient with poor bone quality; all other cases showed uneventful bone healing.

Conclusions:

  • Intraoperative IM nail customization is a valuable technique for precise alignment control in orthopedic surgery.
  • The sterile bending device is safe, easy to handle, and capable of bending various IM nails, expanding IM nailing indications to include complex deformities.
  • Consideration of future implant removal is necessary when determining the bending site.
