Background:

  • Taeniasis is a human intestinal infection caused by tapeworms.
  • Taenia solium and Taenia saginata are common causes, but Taenia asiatica is increasingly recognized, particularly in Asia.
  • Understanding the specific tapeworm species is crucial for accurate diagnosis and treatment.

Observation:

  • A 46-year-old Indonesian male presented with a decade-long history of daily passage of mobile tapeworm segments (proglottids).
  • Physical examination revealed no significant symptoms, despite the chronic nature of the discharge.
  • A 2.86-meter-long tapeworm was recovered after treatment with Praziquantel and Bisacodyl.

Findings:

  • Microscopic examination of the recovered tapeworm revealed unique morphological characteristics, including 16 pairs of uterine branches and the absence of a rostellum on the scolex.
  • These features, along with the patient's ethnographical background involving consumption of raw pork, strongly indicated Taenia asiatica infection.
  • The proglottid morphology (length:width ratio of 1-1.5:1) further differentiated it from T. solium and T. saginata.

Implications:

  • This case highlights the importance of considering Taenia asiatica in the differential diagnosis of taeniasis in endemic regions.
  • The unique morphological findings underscore the need for detailed macroscopic and microscopic analysis for precise species identification.
  • Further molecular studies are recommended to fully characterize this tapeworm and its potential subtypes.
