    Background:

    • Alpha-1-antitrypsin (AAT) is a crucial proteinase inhibitor.
    • Understanding AAT levels in newborns is important for assessing neonatal health.

    Purpose of the Study:

    • To determine alpha-1-antitrypsin (AAT) concentrations and phenotypes in healthy newborns.
    • To investigate changes in AAT levels during the early neonatal period.

    Main Methods:

    • Plasma samples were collected from 257 healthy newborns at two time points: within 3 days of birth and around 30 days postpartum.
    • AAT concentrations and Pi phenotypes were analyzed for each sample.

    Main Results:

    • AAT levels were significantly higher in newborns immediately after birth compared to cord blood and 30-day samples for Pi phenotypes M, M1M2, and MS (p < 0.001).
    • Cord blood AAT concentrations were within the normal adult range.
    • Newborns with the PiMZ phenotype exhibited consistently lower AAT levels without significant changes between the two time points.

    Conclusions:

    • Healthy infants show a significant increase in plasma AAT levels shortly after birth, potentially due to neonatal stimuli or the delivery process.
    • Individuals with the PiMZ phenotype may have a limited or absent response to these factors, maintaining characteristically reduced AAT levels.
