Background:

  • Syphilis co-infection in HIV patients presents diagnostic and therapeutic challenges.
  • Neuroretinitis syphilis in HIV-positive individuals requires optimized management strategies.

Observation:

  • A 53-year-old male with HIV presented with blurry vision, optic disc swelling, and vitreous opacities.
  • Ocular examination revealed reduced visual acuity, afferent pupillary defect, and neural layer thickening on OCT.
  • VDRL and TPHA titers were high, but cerebrospinal fluid VDRL was non-reactive.

Findings:

  • The patient was diagnosed with left-sided neuroretinitis secondary to syphilis in HIV stage II on HAART.
  • Treatment included topical steroids, neurotropic vitamins, and Benzathine Penicillin G.
  • Visual acuity improved, and serological markers decreased after 2 months of treatment.

Implications:

  • Accurate screening for syphilis is vital due to diverse clinical presentations.
  • Ocular syphilis in HIV patients carries an increased risk of neurological complications.
  • Standard treatment regimens may have a higher risk of failure in HIV-positive individuals with ocular syphilis.
