Tertiary syphilis can present as a rare penile gumma, mimicking cancer. Early biopsy is crucial for accurate diagnosis and treatment of this uncommon condition.

Area of Science:

  • Urology
  • Infectious Diseases
  • Dermatology

Background:

  • Gummatous syphilis of the penis is an exceptionally rare manifestation of tertiary syphilis.
  • It can present as a penile lesion, often mistaken for penile carcinoma.

Observation:

  • A 56-year-old male presented with a painful penile lesion and inguinal lymphadenopathy.
  • Imaging suggested penile carcinoma, but histopathology confirmed tertiary syphilis (gummatous syphilis).

Findings:

  • The penile lesion showed a granulomatous inflammatory infiltrate consistent with syphilis.
  • The patient was successfully treated with intravenous penicillin, showing clinical and serologic improvement.

Implications:

  • Penile gumma, though rare, must be included in the differential diagnosis of penile masses.
