Insulin Formulations: Types and Delivery01:27

Insulin Formulations: Types and Delivery

666
Insulin preparations are categorized by their duration of action into short-acting and long-acting types. Two strategies are used to modify insulin's absorption and pharmacokinetic profile: slowing the absorption post-subcutaneous injection, or altering human insulin's amino acid sequence or protein structure. These changes retain the insulin's ability to bind to the insulin receptor, but alter its behavior in solution or after injection.
Short-acting insulins are divided into...

Formulating and Validating Nursing Diagnosis II

3.6K
Nursing diagnoses represent a problem validated by major defining characteristics. There are four categories of nursing diagnoses: problem-focused, risk, health promotion or wellness, and syndrome. The anatomy of a nursing diagnosis includes three components: problem statement or diagnostic label, defining characteristics, and related factors.
Risk nursing diagnoses represent clinical judgments of an individual, family, or community more vulnerable to developing the health problem than others...

Moment of a Force: Scalar Formulation01:18

Moment of a Force: Scalar Formulation

3.1K
The moment of a force, also known as torque, measures the ability of the force to create rotational motion in a body about an axis. It is a vector quantity, meaning it has both magnitude and direction. This concept is used extensively in engineering, physics, and mechanics.
Consider a simple example of a flywheel being rotated about a point, O, by applying a force to it. In this case, the moment arm is the perpendicular distance between the point O and the line of action of the force. The...

Resultant Moment: Scalar Formulation

2.2K
When multiple forces act on an object in two-dimensional space, the concept of the net moment can be used to understand the tendency of these forces to induce rotational motion about a fixed point. The scalar formulation of the resultant moment is a helpful tool in analyzing the equilibrium of structures subjected to multiple forces.
To determine the resultant moment, the moments caused by all the forces in a system in the x-y plane are considered. Positive moments are typically...

Moment of a Force: Vector Formulation

6.4K
The moment of force refers to the measure of the rotational tendency of a force. It occurs when a force is applied in such a way that it produces a twisting or rotational motion rather than linear motion. The moment arm of a force is the perpendicular distance from the line of action of the force to the axis of rotation. The moment of force is not a scalar but a vector quantity.
The vector formulation of the moment of force is the cross-product of the position and force vectors. The...

Formulating and Validating Nursing Diagnosis I

3.6K
A nursing diagnosis is written when the nurse recognizes a cluster of essential patient data indicating health problems treated with independent nursing interventions. The standardized terminologies of a nursing diagnosis help nurses identify and treat patients' problems. Every electronic health record that uses nursing diagnosis must employ standard diagnostic terminology. Developing an efficient, individualized care plan begins with accurate nursing diagnoses.
There are thirteen domains...

Area of Science:

  • Pediatric Anesthesiology
  • Cardiovascular Physiology
  • Pharmacology

Background:

  • Propofol is a common anesthetic agent used in pediatric procedural sedation.
  • Concerns exist regarding propofol's potential cardiotoxicity, especially in vulnerable infant populations.
  • The impact of propofol formulation (diluted vs. undiluted) on infant cardiac function requires further investigation.

Purpose of the Study:

  • To compare the effects of undiluted 1% propofol versus a diluted formulation on infant cardiac function.
  • To assess myocardial deformation using tissue Doppler imaging (TDI) and peak longitudinal strain (LS).
  • To determine if propofol dilution can mitigate cardiac dysfunction during sedation in infants.

Main Methods:

  • Prospective study involving infants over 30 days old undergoing central line insertion.
  • Two groups: undiluted 1% propofol (P1%) and diluted propofol (Pd) (1:1 with 0.9% NaCl).
  • Echocardiograms (TDI, LS) performed pre-sedation, immediately post-sedation, and 1-hour post-sedation.

Main Results:

  • Undiluted propofol (P1%) significantly reduced TDI velocities and LS at immediate and 1-hour post-administration.
  • Diluted propofol (Pd) caused transient reduction in left ventricular TDI velocities only at immediate post-administration, with recovery by 1-hour.
  • No significant difference in LS was observed in the diluted group.

Conclusions:

  • Diluting propofol with 0.9% NaCl may attenuate myocardial dysfunction in infants undergoing sedation.
  • Propofol formulation appears critical in preserving cardiac function in pediatric patients.
  • Further research is warranted to confirm the safety and efficacy of diluted propofol for pediatric sedation.
