    Summary

    Levamisole, an immunomodulator, showed partial efficacy in treating lupus nephritis in one of two systemic lupus erythematosus patients. The other patient responded to cyclophosphamide, indicating varied treatment responses.

    Background:

    • Systemic lupus erythematosus (SLE) is an autoimmune disease that can affect multiple organs.
    • Lupus nephritis, a severe complication of SLE, involves kidney inflammation and damage.
    • Current treatments for lupus nephritis have limitations, necessitating exploration of novel therapeutic agents.

    Observation:

    • Two patients with SLE and lupus nephritis were treated with levamisole, an immunomodulator.
    • One patient exhibited significant clinical and immunological improvements.
    • The second patient did not respond to levamisole but showed positive response to cyclophosphamide.

    Findings:

    • Levamisole treatment led to restoration of urinary protein excretion and creatine clearance in one patient.
    • Immunological markers, including antinuclear antibodies (ANA), Ig-bearing cell number, and PHA-skin test, normalized in the responsive patient.
    • The differential response highlights the heterogeneity of lupus nephritis and potential variability in treatment efficacy.

    Implications:

    • Levamisole may be a potential therapeutic option for select patients with lupus nephritis.
    • Cyclophosphamide remains an effective treatment for refractory cases.
    • Further research is warranted to identify predictive markers for levamisole response in lupus nephritis.
