Background:

  • Heparin is standard for preventing clots during extracorporeal circulation (ECC).
  • Developing heparin-free ECC methods is crucial for patients with heparin-induced complications.

Purpose of the Study:

  • To evaluate the efficacy of heparin-free extracorporeal membrane oxygenator (ECMO) circuits.
  • To compare standard heparin anticoagulation with no anticoagulation and an albumin-heparin surface coating.

Main Methods:

  • Veno-arterial ECMO was performed on 15 rabbits for 5 hours.
  • Groups included standard heparin, no anticoagulation, and albumin-heparin coated surfaces.
  • Activated clotting time, platelet count, blood gas, thrombosis, and histological emboli were assessed.

Main Results:

  • No statistically significant differences were found in quantitative parameters across groups.
  • Thrombus formation occurred in non-heparinized systems, but was inhibited by albumin coating in high-flow areas.
  • No significant benefit of albumin-heparin coating or systemic heparinization was observed in short-term ECC.

Conclusions:

  • Short-term extracorporeal circulation without heparin may be feasible.
  • Neither systemic heparin nor albumin-heparin coating demonstrated a clear advantage in this rabbit model.
  • Further research is needed to optimize heparin-free ECC strategies.
