Cognitive behavioral therapy (CBT) effectively treated postpartum panic disorder (PD) in three Japanese women. This approach is feasible and beneficial for mothers experiencing PD after childbirth.
Area of Science:
- Psychiatry
- Clinical Psychology
- Women's Health
Background:
- Perinatal anxiety is prevalent, with postpartum panic disorder (PD) affecting 10-30% of women.
- Postpartum PD is significantly more common than in the general population (1.5-3%).
- Existing research on cognitive behavioral therapy (CBT) for postpartum PD, especially in Japanese women, is limited.
Purpose of the Study:
- To investigate the effectiveness of CBT in treating postpartum PD among Japanese women.
- To document symptom improvement in postpartum patients receiving CBT.
Main Methods:
- Three married women in their thirties diagnosed with PD and agoraphobia participated.
- The Mini-International Neuropsychiatric Interview and Panic Disorder Severity Scale (PDSS) were used for diagnosis and symptom assessment.
- All patients received 16 weekly 50-minute sessions of CBT and completed the treatment.
Main Results:
- Patients exhibited preoccupation with maternal protective instincts, linking perinatal symptoms to childcare fears.
- All participants showed decreased panic symptoms post-CBT, as measured by the PDSS.
- Two patients no longer met clinical criteria for PD after treatment (e.g., PDSS scores reduced from 13 to 3, 22 to 6, and 7 to 1).
Conclusions:
- CBT demonstrates a therapeutic effect and feasibility for treating postpartum PD.
- Therapeutic interventions should consider incorporating child-inclusive activities for enhanced maternal well-being.
