Background:

  • Endogenous hyperinsulinism (EI) is characterized by excessive insulin secretion, often caused by insulinoma (55% of cases).
  • Alternative causes include islet cell hyperplasia, nesidioblastosis, or autoantibodies against insulin or its receptor.
  • Differentiating these conditions poses diagnostic challenges despite various morphological examinations.

Observation:

  • A study reviewed six patients operated for EI between 2000 and 2015.
  • Causes included insulinoma (3 cases), endocrine cell hyperplasia (2 cases), and one case with no identifiable lesion.
  • Patients presented with typical adrenergic and neuroglycopenic symptoms, low blood glucose, and elevated insulin/C-peptide levels.

Findings:

  • Imaging modalities showed variable efficacy: CT identified insulinoma in 1/2 cases, MRI in 2/2 cases.
  • Endoscopic ultrasound yielded one correct insulinoma localization but three false positives out of four.
  • Intraoperative ultrasound localized insulinoma in 2/4 cases, with two false positives.

Implications:

  • Diagnostic challenges persist in EI, even with suggestive imaging findings.
  • Considering differential diagnoses beyond insulinoma is crucial for appropriate patient management.
  • Invasive diagnostic explorations may be necessary when non-invasive methods are inconclusive.
