  • Papillary thyroid carcinoma (PTC) is the most common type of thyroid cancer.
  • Locally invasive PTC typically presents with compressive symptoms or palpable neck masses.
  • Infection-induced presentation of PTC is exceptionally rare.

Observation:

  • A 45-year-old woman presented with a rapidly enlarging, inflamed anterior cervical swelling.
  • Clinical and imaging studies revealed a large, multi-cystic thyroid mass with significant local invasion.
  • Fine needle aspiration biopsy yielded purulent material, initially suggesting infection over malignancy.

Findings:

  • Surgical exploration confirmed a large (11cm) papillary thyroid carcinoma with extensive local invasion.
  • The tumor infiltrated the trachea, vascular axis, and adjacent muscles and nerves.
  • Histopathological examination confirmed papillary cystic carcinoma, with subacute infection as the initial revealing factor.

Implications:

  • This case underscores the possibility of infection mimicking or triggering the presentation of advanced thyroid cancer.
  • Early and accurate diagnosis is crucial, even with atypical presentations, to guide appropriate treatment.
  • Highlights the importance of considering rare differential diagnoses in complex clinical scenarios.
