    Background:

    • Primary headaches, including migraine and tension-type headache (TTH), are prevalent conditions.
    • Sleep disturbances are frequently anecdotally linked to headache disorders.
    • A systematic evaluation is needed to quantify the relationship between specific sleep issues and primary headaches.

    Purpose of the Study:

    • To investigate the association between sleep disturbances and primary headache disorders.
    • To compare sleep quality and insomnia prevalence in patients with TTH, migraine, and healthy controls.

    Main Methods:

    • Prospective study comparing patients with active primary headaches (TTH and migraine) to a control group.
    • Utilized standardized questionnaires: Insomnia Severity Index (ISI), Pittsburgh Sleep Quality Index (PSQI), Epworth Sleepiness Scale (ESS), and Berlin Sleep Apnea Questionnaire (BSAQ).
    • Compared questionnaire results across TTH, migraine, and control groups.

    Main Results:

    • Patients with TTH showed significantly higher rates of insomnia (75% vs. 50% migraine, 37.3% controls; P=.002).
    • Poor sleep quality was most prevalent in the TTH group (87.5% vs. 47.7% migraine, 43.3% controls; P=.0001).
    • TTH patients (53.1%) were more likely to experience insufficient sleep than migraine (25%) or control (29.9%) groups (P=.025).

    Conclusions:

    • Tension-type headache patients exhibit a significantly higher likelihood of insomnia compared to migraine patients and controls.
    • A substantial majority of TTH patients reported poor sleep quality, with about half of migraine and control individuals also affected.
    • Inadequate sleep was identified as a headache trigger by three-quarters of TTH patients and over half of migraine patients.
