Background:

  • Lower urinary tract symptoms (LUTS) can be challenging to diagnose when standard urodynamics (UDS) are inconclusive.
  • Ambulatory urodynamics (AUDS) offers continuous monitoring to capture symptoms in a natural setting.

Purpose of the Study:

  • To evaluate the added diagnostic and management value of AUDS in patients with unexplained LUTS.
  • To determine the impact of AUDS findings on treatment strategies.

Main Methods:

  • Retrospective review of urodynamic data from January 2006 to December 2018 at a tertiary referral center.
  • Analysis of 430 patients who underwent AUDS due to inconclusive standard or video UDS results.
  • Inclusion of 391 patients (360 female, 31 male) in the final analysis.

Main Results:

  • Symptoms were reproduced in 74% of patients undergoing AUDS.
  • Common findings included detrusor overactivity (with or without incontinence) and urodynamic stress incontinence.
  • AUDS provided additional information leading to a change in patient management in 75.7% of cases.

Conclusions:

  • AUDS monitoring yields significant diagnostic findings in the majority of patients with unexplained LUTS.
  • The information obtained from AUDS directly influences and guides the development of new treatment plans.
  • AUDS is a valuable supplementary diagnostic tool for complex lower urinary tract symptom evaluation.
