Summary

An extended thenar flap effectively reconstructs two adjacent fingertip amputations, showing excellent flap survival and functional recovery. This surgical technique offers a promising solution for complex hand injuries with no reported complications.

Background:

  • The thenar flap is a recognized surgical method for fingertip reconstruction.
  • Limited data exists on its application for multiple, adjacent fingertip injuries.

Purpose of the Study:

  • To introduce an extended thenar flap technique for simultaneous reconstruction of two adjacent fingertip amputations.
  • To evaluate the clinical outcomes and complications associated with this technique.

Main Methods:

  • A retrospective study of 12 patients (24 fingers) who underwent extended thenar flap reconstruction for adjacent fingertip amputations between October 2013 and October 2016.
  • Assessment included flap survival, range of motion (ROM), two-point discrimination (2PD), quick disabilities of the arm, shoulder, and hand (DASH) score, Michigan Hand Outcome Questionnaire (MHQ) score, and return to work time.

Main Results:

  • All 12 extended thenar flaps survived successfully.
  • Patients demonstrated good functional recovery with a mean total active ROM of 255°, mean static 2PD of 6.9 mm, mean quick DASH score of 3.3, and mean MHQ score of 93.8.
  • All patients returned to work within an average of 6.2 weeks, with no reported complications.

Conclusions:

  • The extended thenar flap is a viable and effective surgical option for covering defects from two adjacent fingertip amputations.
  • This technique provides satisfactory functional and aesthetic outcomes with a low complication rate.
