Murine Intrapulmonary Tracheal Transplantation: A Model for Investigating Obliterative Airway Disease After Lung Transplantation

1.4K
The murine intrapulmonary tracheal transplantation (IPTT) model is valuable for studying obliterative airway disease (OAD) after lung transplantation. It offers insights into lung-specific immunological and angiogenic behavior in airway obliteration after allotransplantation with high reproducibility. Here, we describe the IPTT procedure and its expected...

Pericarditis I: Introduction01:22

Pericarditis I: Introduction

352
Pericarditis is defined as the inflammation of the pericardium, the thin, sac-like membrane surrounding the heart. This condition can cause significant chest pain and other symptoms, often necessitating medical intervention. The pericardium has two layers: the inner visceral layer and the outer parietal layer, separated by a small amount of fluid that reduces friction during heartbeats.Types of PericarditisPericarditis can be classified into several types based on the duration and nature of the...

Summary
This summary is machine-generated.

Constrictive pericarditis is a rare complication after lung transplantation. Early diagnosis and pericardiectomy are key for successful treatment in these patients.

Area of Science:

  • Cardiology
  • Thoracic Surgery
  • Pulmonology

Background:

  • Constrictive pericarditis is an increasingly recognized long-term complication following lung transplantation.
  • Awareness of this pathology has grown due to improved diagnostics and treatment.

Purpose of the Study:

  • To investigate the causes and natural history of constrictive pericarditis after lung transplantation.
  • To present institutional experience with this rare complication.

Main Methods:

  • Reviewed institutional database of 1234 lung transplant recipients from October 2005 to October 2018.
  • Identified 10 patients (0.8%) who developed constrictive pericarditis at a median of 11.2 months post-transplant.
  • Used descriptive statistics to analyze patient data and associated variables.

Main Results:

  • 8 of 10 patients had idiopathic pulmonary fibrosis as the indication for transplant.
  • All patients presented with dyspnea and pleural effusions; constrictive physiology was confirmed by right heart catheterization.
  • Eight patients underwent pericardiectomy, showing improved hemodynamics and symptom resolution with no 30-day mortality.

Conclusions:

  • Consider constrictive pericarditis in lung transplant patients with new heart failure or pleural effusions within 2 years.
  • Idiopathic pulmonary fibrosis may increase the risk of developing constrictive pericarditis post-transplant.
  • Pericardiectomy is a safe and effective treatment for post-transplant constrictive pericarditis.
