The Langenskiöld classification for Blount disease (BD) shows excellent reliability among orthopedic specialists. Surgical experience does not impact the accuracy of this diagnostic tool for genu varum.
Area of Science:
- Orthopedics
- Pediatric Orthopedics
- Radiology
Background:
- Blount disease (BD) is a developmental condition causing genu varum due to medial proximal tibial physis issues.
- Plain radiographs are essential for diagnosing BD.
- The Langenskiöld classification is a standard diagnostic tool for BD.
Purpose of the Study:
- To assess the intra- and inter-observer reliability of the Langenskiöld classification for Blount disease.
- To address the lack of data on the Langenskiöld classification's reliability in English literature.
Main Methods:
- A cross-sectional study included 37 patients with Blount disease.
- Three groups (residents, surgeons, pediatric orthopedic surgeons) evaluated clinical findings and radiographs.
- Intraclass correlation coefficient (ICC) tests were used to determine intra- and inter-observer reliability.
Main Results:
- Intraobserver reliability was excellent across all three groups (ICC: 0.822, 0.804, 0.763).
- Interobserver reliability among the groups was also found to be excellent.
- Agreement with Langenskiöld classifications was excellent for all evaluated groups.
Conclusions:
- The Langenskiöld classification demonstrates excellent intra- and inter-observer reliability in diagnosing Blount disease.
- Surgical experience did not significantly alter the reliability of the Langenskiöld classification.
- This study supports the consistent application of the Langenskiöld classification in clinical practice.
