Background:

  • A 59-year-old male with tonsillar cancer and radiation chondronecrosis underwent therapeutic hypothermia.
  • The patient experienced respiratory and cardiac arrest post-laryngoscopy, necessitating initial targeted temperature management (TTM).

Observation:

  • Following rewarming, the patient developed upper airway obstruction and a second prolonged cardiac arrest.
  • Return of spontaneous circulation (ROSC) was achieved after 45 minutes of resuscitation.

Findings:

  • Sequential TTM was initiated again, with the patient cooled to 32°C for 48 hours.
  • The patient recovered with intact neurological function five days after the second cooling period.

Implications:

  • This case underscores the neuroprotective potential of sequential TTM following recurrent cardiac arrest and ROSC.
  • Sequential TTM may be a vital treatment strategy in critical care settings for preserving neurological function after severe ischemic events.
