    Summary

    Malignant phyllodes tumors are rare in men. This case highlights aggressive local recurrence and distant metastasis, emphasizing the need for prompt surgical management and close follow-up for these rare breast tumors.

    Background:

    • Phyllodes tumors are rare breast neoplasms with benign, borderline, and malignant subtypes.
    • While uncommon in males, phyllodes tumors can occur and present aggressive behavior.

    Observation:

    • A 73-year-old male presented with a massive left breast tumor.
    • Initial biopsy suggested malignancy; CT confirmed a large chest wall mass.
    • Diagnosis was high-grade malignant phyllodes tumor post-mastectomy.

    Findings:

    • The patient developed rapid local recurrence and bilateral pulmonary metastases.
    • Malignant phyllodes tumors show a propensity for hematogenous spread, primarily to the lungs.
    • Recurrence is common, even after complete excision, especially in malignant cases.
