Patients with idiopathic pulmonary fibrosis (IPF) on pirfenidone may face an increased risk of developing tuberculosis (TB). A rare skin reaction, exfoliative dermatitis, occurred due to pirfenidone and rifampicin interaction.

  • Idiopathic pulmonary fibrosis (IPF) is a progressive lung disease.
  • Pirfenidone is an antifibrotic medication used to treat IPF.

Observation:

  • A patient diagnosed with IPF and treated with pirfenidone developed TB.
  • The patient later experienced exfoliative dermatitis, a severe skin reaction.
  • This dermatitis was attributed to an interaction between pirfenidone and rifampicin, an anti-TB drug.

Findings:

  • This case suggests a potential increased risk of TB in IPF patients receiving antifibrotic therapy.
  • A previously undocumented adverse drug reaction involving pirfenidone and rifampicin was observed.
