Background:

  • Traumatic brain injury (TBI) is a major cause of long-term disability, with persistent cognitive and behavioral symptoms.
  • Current strategies for mitigating chronic TBI symptoms are limited.
  • Disruption of metabolic pathways is a key feature of TBI-induced damage.

Purpose of the Study:

  • To investigate if modulating metabolic activity can restore behavioral performance in chronic TBI.
  • To test the efficacy of pioglitazone, an FDA-approved diabetes drug, in treating cognitive deficits after TBI.

Main Methods:

  • Rats with lateral fluid percussion injury received pioglitazone treatment starting 46 days post-injury.
  • Working memory performance was assessed using the radial arm maze (RAM) after 14 days of treatment.
  • Hippocampal hexokinase activity, glucose transporter, and glial marker expression were analyzed.

Main Results:

  • Pioglitazone-treated TBI rats showed significantly improved working memory performance in the RAM compared to untreated TBI rats.
  • Treated rats performed similarly to control animals, indicating functional recovery.

Conclusions:

  • Targeting brain metabolism, specifically in the hippocampus, shows promise for alleviating cognitive symptoms in chronic TBI.

  • Further research into metabolic modulation strategies for TBI is warranted.
