Summary

Children with idiopathic nephrotic syndrome (NS) are not prone to splenic hypofunction. This study found normal splenic function in children with NS, challenging previous theories linking infections to spleen issues in these patients.

Background:

  • A prior hypothesis suggested splenic hypofunction as the cause of this immunological defect in NS patients.

Purpose of the Study:

  • To investigate the splenic function in children diagnosed with idiopathic nephrotic syndrome (NS).
  • To determine if splenic hypofunction contributes to the increased infection risk observed in pediatric NS.

Main Methods:

  • Splenic function was assessed by quantifying circulating pocked red blood cells (PkRBCs) via interference phase contrast microscopy.
  • PkRBC levels were measured in 19 children with NS (33 measurements total).
  • Normal individuals have <2% PkRBCs, asplenic individuals have 15-30%, and hyposplenic individuals have intermediate levels.

  • All tested children with NS demonstrated normal PkRBC levels, ranging from 0% to 0.8%.
  • This finding held true even for two patients experiencing bacterial peritonitis during a relapse.
