Purpose of the Study:

  • To assess the annual rate of decline in forced expiratory volume in 1 second (FEV1) in a Korean community cohort.
  • To examine the relationship between FEV1 decline and smoking status, sex, and pre-bronchodilator airflow limitation.

Main Methods:

  • Analysis of 5,865 subjects aged 40-69 from the Ansung-Ansan cohort (2001-2006).
  • Utilized a generalized additive mixed model to assess annual FEV1 decline over a mean follow-up of 3.8 years.
  • Stratified analysis by smoking status, sex, and presence of airflow limitation.

Main Results:

  • Overall, men showed a significantly more rapid annual FEV1 decline than women (31.3 mL vs 27.0 mL).
  • In men without airflow limitation, current smokers experienced the fastest decline (40.1 mL/year) compared to never smokers (31.5 mL/year).
  • Smoking cessation was associated with a slower FEV1 decline in men, irrespective of airflow limitation.

Conclusions:

  • Smoking accelerates lung function decline in Korean males.
  • Smoking cessation is vital for slowing FEV1 decline in Korean men, regardless of pre-existing airflow limitation.
  • Public health strategies should emphasize smoking cessation programs for Korean men to preserve lung function.
