    Background:

    • Acute B cell lymphocytic leukaemia (B-ALL) is a significant hematologic malignancy.
    • Understanding the immunophenotypic profile of leukaemic cells is crucial for diagnosis and treatment.
    • Serial analysis of surface markers can reveal dynamic changes in leukaemic cell populations.

    Observation:

    • Two cases of B-ALL were analyzed for surface marker expression.
    • Leukaemic lymphocytes initially expressed the mouse erythrocyte receptor (MER) and surface immunoglobulin (SIg) with lambda light chain.
    • Changes in MER expression, SIg density, and acquisition of SIgA were noted during disease progression and treatment.

    Findings:

    • Leukaemic cells demonstrated alterations in surface marker expression over time.
    • These alterations were consistent with increased immunological differentiation.
    • Specific changes included loss of MER, increased SIg density, and gain of SIgA.
