Area of Science:

  • Pediatric Oncology
  • Oral Pathology
  • Histiocytosis

Background:

  • Eosinophilic granuloma (EG) is a localized form of Langerhans cell histiocytosis (LCH).
  • EG typically affects bone but can manifest in soft tissues, including the oral cavity.
  • Oral EG is rare, particularly in infants, presenting diagnostic and therapeutic challenges.

Observation:

  • A 1.5-year-old Kuwaiti male child presented with extensive bilateral ulcerative lesions of the palate and gingiva.
  • Massive submandibular lymphadenopathy was noted concurrently.
  • Initial bone surveys were unremarkable, suggesting a soft tissue-predominant presentation.

Findings:

  • Histological examination confirmed the diagnosis of eosinophilic granuloma.
  • The patient received a combination of chemotherapy and localized irradiation.
  • A dramatic clinical response was observed following treatment.

Implications:

  • This case demonstrates the efficacy of multimodal therapy in managing aggressive oral eosinophilic granuloma in a pediatric patient.
  • Early diagnosis and prompt treatment are crucial for favorable outcomes and preventing relapse.
  • Further research into optimal treatment strategies for rare LCH variants is warranted.
