Costo-iliac impingement syndrome.

    Summary

    This study describes a syndrome where the lowest rib irritates the iliac crest, causing back and loin pain. Surgical removal of the affected rib relieved symptoms in all six patients, preventing recurrence.

    Background:

    • Chronic back and loin pain can significantly impact quality of life.
    • Understanding the anatomical causes of such pain is crucial for effective treatment.

    Observation:

    • Six patients presented with back and loin pain attributed to impingement of the lowest rib against the iliac crest.
    • These patients also exhibited significant height loss and, in five cases, vertebral osteoporosis.

    Findings:

    • Subperiosteal resection of the outer two-thirds of the affected (twelfth) rib successfully relieved pain in all six patients.
    • One patient also required resection of the eleventh rib.
    • Symptomatic relief was sustained for 5 to 34 months postoperatively without recurrence.

    Implications:

    • This surgical approach offers a potential solution for patients suffering from rib-iliac crest impingement syndrome.
    • It highlights the importance of considering lower rib anomalies in the differential diagnosis of chronic back pain.
