Background:

  • The selection of appropriate final-impression techniques and materials is crucial for the successful fabrication of complete dentures (CD) and removable partial dentures (RPD).
  • Existing research comparing these techniques and materials is limited and often of insufficient quality to guide clinical practice.

Purpose of the Study:

  • To systematically review and synthesize the evidence from randomized controlled trials (RCTs) comparing different final-impression techniques and materials for complete dentures (CD) and removable partial dentures (RPD).
  • To assess the impact of various impression methods on patient-reported outcomes and satisfaction.

Main Methods:

  • A comprehensive literature search was conducted across multiple databases, including Cochrane Oral Health's Trials Register, CENTRAL, Medline, Embase, ClinicalTrials.gov, and WHO ICTRP.
  • Randomized controlled trials (RCTs) comparing different final-impression techniques and materials for CD and RPD were included. Data extraction and risk of bias assessment were performed independently by two reviewers.
  • Meta-analysis was employed using a random-effects model to pool dichotomous and continuous outcomes, expressed as risk ratios (RR) and mean differences (MD) or standardized mean differences (SMD) with 95% confidence intervals (CI).

Main Results:

  • Nine RCTs were included, with eight focusing on complete dentures (CD).
  • For CD, low-quality evidence suggested silicone may be superior to alginate for oral health-related quality of life. Very low-quality evidence indicated no clear difference between single-stage alginate and two-stage elastomer impressions for participant-reported quality of life (OHIP-EDENT).
  • For removable partial dentures (RPD), low-quality evidence showed no significant difference in general satisfaction between altered-cast and one-piece cast techniques at one-year follow-up.

Conclusions:

  • There is currently insufficient high-quality evidence to support a substantial advantage of any specific final-impression technique or material for fabricating complete dentures (CD) and removable partial dentures (RPD).
  • The available evidence is limited and of low to very low quality, highlighting a significant gap in knowledge.
  • Further high-quality randomized controlled trials (RCTs) are urgently needed to provide robust data for clinical decision-making in prosthodontics.
