Summary

Carotid webs are more common in young adults with cryptogenic strokes. These webs may contribute to stroke risk and are associated with intracranial large vessel occlusions.

Background:

Purpose of the Study:

  • To determine the frequency of carotid webs in young patients with cryptogenic ischemic strokes.
  • To compare the prevalence of carotid webs in cryptogenic stroke patients to a control group.

Main Methods:

  • Retrospective analysis of computed tomography (CT) angiography neck images from cryptogenic ischemic stroke patients and trauma controls (aged 18-60).
  • Carotid webs were defined as shelf-like defects in the internal carotid artery bulb.
  • Image evaluation was performed independently by two fellowship-trained specialists.

Main Results:

  • Carotid webs were identified in 25% of cryptogenic stroke patients versus 0% in the control group (P < .001).
  • Among cryptogenic stroke patients with anterior circulation infarcts, 23% had ipsilateral carotid webs.
  • Patients with carotid webs showed a higher incidence of proximal large vessel occlusions (P = .04).

Conclusions:

  • Carotid webs are significantly more frequent in young patients with cryptogenic ischemic strokes.
  • The findings suggest a potential thrombogenic role for carotid webs in young stroke patients.
  • Symptomatic carotid webs are associated with a higher likelihood of intracranial large vessel occlusions.
