    Area of Science:

    • Dermatology
    • Pharmacology

    Background:

    • Severe cystic acne and rosacea are often resistant to conventional treatments.
    • Isotretinoin is a synthetic oral retinoid with novel therapeutic properties.

    Purpose of the Study:

    • To evaluate the efficacy of isotretinoin in treating severe recalcitrant cystic acne and rosacea.
    • To assess the pharmacological effects and side effect profile of isotretinoin.

    Main Methods:

    • Oral administration of isotretinoin at dosages ranging from 0.5 to 1.0 mg/kg body weight daily.
    • Observation of treatment outcomes in patients with severe cystic acne and rosacea.

    Main Results:

    • Significant reduction in inflammatory skin eruptions observed.
    • Long-lasting remission achieved after drug discontinuation.
    • Satisfactory results reported in 18 patients with rosacea.
    • Mucocutaneous side effects were common but did not necessitate treatment cessation.

    Conclusions:

    • Isotretinoin provides a new therapeutic option for severe nodulocystic acne and recalcitrant rosacea.
    • The drug's mechanism involves decreased sebaceous gland size, reduced sebum, and altered bacterial populations.
    • Contraindicated in pregnancy due to teratogenicity.
