Background:

  • Pulmonary segmentectomy is a lung-sparing surgical procedure.
  • Advancements in robotic technology offer new possibilities for minimally invasive thoracic surgery.

Purpose of the Study:

  • To report initial outcomes of pulmonary robotic segmentectomy.
  • To describe the surgical technique, portal positioning, and patient results.

Main Methods:

  • Prospective data collection from a robotic surgery database (January 2017 - December 2018).
  • Patients underwent robotic segmentectomy using the Da Vinci system with a three-portal technique.
  • Individual dissection of hilar structures, segmental vessel/bronchus ligation, and stapled parenchymal transection were performed.

Main Results:

  • Forty-nine patients (33 female, average age 68) underwent robotic segmentectomy for lung cancer or benign disease.
  • No conversions to open or laparoscopic surgery, or lobectomy, were required.
  • Median operative time was 160 minutes; median console time was 117 minutes. Postoperative complications occurred in 18.3% of patients, with 14.2% experiencing prolonged hospitalization.

Conclusions:

  • Robotic segmentectomy is a safe and viable surgical option for lung conditions.
  • The procedure is associated with a short hospital stay and low morbidity rates.
