    Summary

    Investigating iron deficiency in adult males and postmenopausal females revealed chronic gastrointestinal blood loss as the primary cause in most cases. Inadequate diet and salicylate use were also contributing factors.

    Background:

    • Iron deficiency anemia is a common condition with various underlying causes.
    • Identifying the etiology is crucial for effective treatment in adult males and postmenopausal females.

    Purpose of the Study:

    • To determine the primary causes of iron deficiency in adult males and postmenopausal females presenting at a general hospital.
    • To assess the diagnostic utility of red cell indices and blood film morphology.

    Main Methods:

    • Prospective study over 15 months.
    • Inclusion criteria: hemoglobin < 10.6 g/dl, mean corpuscular volume < 86 fl; exclusion: anemia post-trauma/surgery.
    • Iron status assessed via serum iron studies, serum ferritin, or sternal marrow aspiration.

    • 215 patients assessed; 104 (48.4%) had iron deficiency.
    • Iron deficiency causes identified in 101 out of 104 patients.

    • Chronic gastrointestinal blood loss is the predominant cause of iron deficiency in this demographic.
    • Serum iron studies, ferritin, and marrow aspiration are essential for diagnosing iron deficiency.
    • Red cell indices and blood film morphology are not definitive diagnostic markers for iron deficiency.
