Summary

Sentinel lymph node biopsy (SLNB) using 99mTc accurately stages prostate cancer, identifying metastases missed by extended lymphadenectomy. This technique offers high sensitivity and 100% negative predictive value for detecting lymphatic spread.

Background:

  • Accurate staging of prostate cancer is crucial for treatment decisions.
  • Sentinel lymph node biopsy (SLNB) is an emerging technique for staging, but its efficacy requires validation.

Purpose of the Study:

  • To validate SLNB against extended pelvic lymphadenectomy (ePLND) for prostate cancer staging in patients with a high Briganti Index (>5).

Main Methods:

  • A prospective longitudinal study involving 84 patients undergoing SLNB followed by ePLND.
  • Radiotracer (99mTc-nanocolloids) and SPECT-CT were used for sentinel node identification.
  • A mixed radiotracer (99mTc + ICG) was used in a subset of patients.

Main Results:

  • SPECT-CT identified radiotracer outside ePLND in 76% of patients.
  • SLNB detected metastases in 28% of patients, with 25% of these found outside the ePLND territory.
  • 99mTc demonstrated 100% sensitivity and 100% negative predictive value (NPV).

Conclusions:

  • SLNB with 99mTc is highly sensitive and has a 100% NPV for detecting lymphatic metastases in prostate cancer.
  • It identifies metastatic nodes missed by ePLND, improving staging accuracy.
  • ICG fluorescence aids visualization but has lower sensitivity and NPV than 99mTc.
