Background:

Trichomonal vaginitis is a common sexually transmitted infection.
Metronidazole is the primary treatment, but resistance is a growing concern.

Observation:

A 36-year-old woman presented with a decade-long history of symptomatic vaginitis.
She underwent 22 treatment courses with metronidazole or tinidazole without sustained relief.

Findings:

The isolated strain of Trichomonas vaginalis demonstrated significantly elevated resistance to metronidazole (MIC 160 µg/ml) compared to typical strains (1.25-10 µg/ml).
The strain also exhibited reduced sensitivity to ornidazole and tinidazole (MIC 80 µg/ml).

Implications:

This case underscores the clinical challenge posed by drug-resistant Trichomonas vaginalis.
Further research is crucial to elucidate the mechanisms of resistance and develop effective therapeutic strategies.
