Summary

Radical lobectomy with lymph node dissection is a standard lung cancer surgery. Systematic nodal dissection (SND) improves intrathoracic staging and patient survival.

Area of Science:

  • Thoracic Surgery
  • Surgical Oncology
  • Pulmonology

Background:

  • Lobectomy with regional lymph node dissection, termed "radical lobectomy", has been the standard lung cancer surgery since 1960.
  • Intrathoracic staging for lung cancer has evolved to include detailed assessment of nodal involvement.
  • Systematic nodal dissection (SND) is recognized by the International Association for the Study of Lung Cancer (IASLC) as crucial for staging.

Purpose of the Study:

  • To describe the significance, recent strategies, and techniques of lymph node dissection in lung cancer surgery.
  • To highlight the evolution of intrathoracic staging in lung cancer.
  • To emphasize the importance of nodal evaluation in lung cancer management.

Main Methods:

  • Systematic nodal dissection (SND) involves excising all tissue within specific anatomical compartments.

  • Techniques include using scissors or electrocautery for dissection.
  • The manuscript reviews historical and current approaches to lymph node management in lung cancer.
  • Main Results:

    • Accurate staging is a primary clinical benefit of lymph node dissection.
    • Improved survival outcomes are associated with thorough lymph node dissection.
    • SND provides a comprehensive assessment of nodal status.

    Conclusions:

    • Lymph node dissection is critical for both accurate staging and survival benefit in lung cancer patients.
    • Systematic nodal dissection represents an advanced and essential component of intrathoracic staging.
    • Continued refinement of lymph node dissection techniques is vital for optimal lung cancer treatment.
