Summary

Nurse-delivered internet-based cognitive behavioral therapy (iCBT) significantly reduced depression and improved quality of life in cardiovascular disease (CVD) patients. This home-based intervention offers an effective treatment option for depression in this population.

Background:

  • Depression frequently impacts well-being and prognosis in cardiovascular disease (CVD) patients.
  • Effective interventions for depression in CVD populations are limited.

Purpose of the Study:

  • To evaluate a nurse-delivered, adapted internet-based cognitive behavioral therapy (iCBT) program for reducing depression in CVD patients.

Main Methods:

  • A randomized controlled trial involved 144 CVD patients with mild depression.
  • Participants were assigned to a 9-week iCBT program or an online discussion forum (ODF).
  • Depression (PHQ-9, MADRS-S), HRQoL (SF-12, EQ-VAS) were primary and secondary outcomes.

Main Results:

  • iCBT showed significant moderate effects on reducing depression (PHQ-9, Cohen d=0.62) versus ODF.
  • iCBT demonstrated significant large effects on MADRS-S (Cohen d=0.86) and moderate effects on SF-12 mental component and EQ-VAS (Cohen d=0.66, 0.62).
  • Module completion rates were 60% for all modules and 82% for at least half.

Conclusions:

  • Nurse-delivered iCBT effectively reduces depression in CVD patients.
  • The intervention improves health-related quality of life (HRQoL) in this population.
  • iCBT provides a convenient, home-based treatment for depression, accessible at the patient's preferred time.
